Intake form and Disclosures

All new clients will be required to fill out an intake form prior to any services. This form collects important information such as:

• Owner's full name and address where services will be preformed

• Pet’s name, breed, weight, and any relevant health details.

• Current rabies vaccination certificate. Veterinarian can be called to confirm the vaccination status directly over the phone while the groomer is present.

• Disclosures of good faith. While injuries are extremely rare, it’s important to remember that when working with unpredictable pets and sharp instruments, accidents can unfortunately happen. We want to assure you that we always strive to provide the utmost care, kindness, and compassion to every pet we handle. By signing this disclosure, you acknowledge and agree that you will not pursue legal action in the unlikely event that such an accident occurs. Your pet’s safety and well-being are our top priorities at all times.

• Mats and De-matting Notice: Mats happen when hair becomes so tangled that it forms dense clumps. These mats can develop due to poor grooming habits, trapped moisture, and constant rubbing against surfaces. While brushing out simple tangles usually doesn’t cause much discomfort, mats can be quite painful and may cause bruises and skin irritation even if you don’t brush them. When multiple mats join together, this condition is known as pelting, which can be extremely painful for your pet. Pelting can also lead to serious complications such as large bruises, severe skin irritation, open sores, infections, and in severe cases, may even result in the loss of limbs if left untreated.

• Treatment of Mats: De-Matting often results in the shortest possible shave to ensure the utmost safety for your pet. Mats tend to form extremely close to the skin and can cause damage to the hair. Since dogs have very thin and delicate skin, pulling or tugging at the mats with clippers significantly increases the risk of cuts, nicks, and irritation. For these reasons, groomers commonly use the shortest length clipper blades available, as they provide the safest and most effective option to carefully remove mats without harming the dog. This method is typically the best approach before professional vet consultation becomes necessary.
